Nacogdoches Memorial Hospital, Trinity Mother Frances Hospitals and Clinics Announce New Clinical Affiliation Agreement


Nacogdoches, TX – Nacogdoches Memorial Health (NMH) and Trinity Mother Frances Hospitals and Clinics (TMFHC) in Tyler have entered into a formal clinical affiliation agreement designed to enhance the quality of care available to patients in their home communities, as well as provide convenient and seamless access to tertiary care for patients and physicians, when needed, at TMFHC.  


TMFHC and NMH have joined together in this agreement in order to expand access to the highest quality care for patients throughout East Texas and to develop the skills and education of clinical staff.

Through this affiliation agreement with TMFHC, NMH will enjoy the best of both worlds – access to the resources of the leading healthcare system in Tyler, while retaining local ownership and leadership in daily operations,” said NMH CEO Scott Street. 


The agreement provides for enhanced access and coordination of care for patients needing specialized or tertiary care outside of the communities served by NMH. This will be facilitated by expedited patient transfers and enhanced use of the Trinity Mother Frances Access Center.


The agreement also helps to ensure the development of clinical programs and services to expand the care available to the communities served by NMH, utilizing the expertise and resources of TMFHC's Clinical Service Lines and Centers of Excellence. Designated clinical staff will also have access to TMFHC training and education programs, including accredited continuing education courses provided by TMFHC physician and nursing experts. The agreement will also provide improved access to information through connectivity to TMFHC’s secure electronic health record (EHR) network to allow NMH medical staff and designated clinical staff access to patient medical records for improved continuity of care.
