Demonstrating Community Need
As part of the Patient Protection and Affordable Care Act, not-for-profit hospitals are required to complete a Community Health Needs Assessment (CHNA) once every three years. In 2013 and again in 2016, a CHNA was conducted and health care needs in each community connected to CHRISTUS Trinity Mother Frances Health System were identified. The CHNA reports are a comprehensive assessment of the identified health needs in Cherokee, Smith and Wood Counties.
The analysis for each hospital includes a careful review of the most current health data available, demographics and input from numerous community representatives. The process culminates in the development of an Implementation Plan to address the significant needs identified through the CHNA. CHRISTUS Trinity Mother Frances Health System will utilize its 2016 CHNAs and Implementation Plans to implement and maintain programs that support its mission and the community.
